1. Two 5-Foot Sections of Small Bubble Wrap, wrapped around MA in BOTH directions
2. Two 3-Foot Sections of Small Bubble Wrap, rolled and places at both ends of box
3. Copy of Symptoms, Complete contact information and any Special Instructions
4. Enough INSURANCE to REPLACE the MA if LOST or DAMAGED*

* If your MA is lost or damaged beyond repair when it arrives at our repair shop, YOU are responsible. This is why we strongly urge you to get Insurance to cover your MA 100% in the (rare) event this happens. It doesn't happen very often, but it HAS HAPPENED!
